Stockton Blvd Safe Stay offers private cabin spaces and provides
person-centered support services to help move program participants into stable housing.
Referrals
The closed referral process will be managed by multi-disciplinary outreach teams (MDTs) deployed to priority encampments and other locations. The initial target area is the W/X corridor, and later on MDTs will deploy to
areas within the vicinity of the Safe Stay Shelter. After the initial list of clients from the W/X corridor is exhausted, referrals will go through 211.
Service Highlights
Participants will work with a case manager throughout the week, fostering a personalized approach. Our case managers assess individual needs, create housing plans, and facilitate essential service connections. Playing a vital role in the stabilization phase, case managers actively collaborate with clients, setting achievable goals for a successful transition out of homelessness.
Many of the individuals we assist are dealing with mental health symptoms rooted in trauma. Understanding the urgency, we are committed to delivering prompt, crisis-oriented services within shelters. Through our partnership with Life Practice Counseling Group, on-site therapy will be available. Sacramento County HEART will conduct weekly mental health assessments, ensuring individuals are connected to mental health services that address their specific needs.
Recognizing the significant medical needs of those we serve, partnering with healthcare providers is essential. By collaborating with community providers, we bring medical care directly to shelter guests, supporting their journey toward stability and well-being.
This program's pet-friendly approach removes a common barrier to accessing shelter. While there is a limit on the number of pets allowed, residents can keep their pets with them in the cabins and use a designated play area. We also partner with community animal care providers to bring essential services directly to the shelter.
